Incident Reports


WCOA is committed to providing an environment that is safe and accessible for all members, volunteers, and staff. Safety includes support during medical emergencies, personal physical safety, physical and verbal abuse, harassment, intimidation, or other threatening disruptive behavior, and control over financial and social affairs. It is the responsibility of each employee to help make WCOA activities and events safe for all of us and treat others with dignity and respect.


1)    When do I fill out an Incident Report?

·       A fall, injury or other medical event to a member, volunteer or WCOA staff person

·       Events affecting personal safety

·       A disruptive outburst of behavior

·       The destruction or loss of property

·       A report should be filled out within 24 hours of the incident


2)    What are some examples that would require an Incident Report Form to be filled out?

·       A fall within or just outside the BSAC, even if paramedics were not called or hospitalization did not occur, a form needs to be filled out.

·       A volunteer driver is bitten by a dog

·       There is a loud disruptive altercation between two members of the BSAC

·       A volunteer or employee cuts or burns themselves at work

·       An employee or volunteer tries to retrieve a box from a shelf and it falls on him or her

·       Something is stolen from WCOA building

·       Fire in the building


NOTE: If a driver has an accident, they will fill out the WCOA Incident Report Involving a Vehicle.


3)    Where do I find an Incident Report form?

·       Click on the form you need below (Incident Report or Incident Report Involving a Vehicle).



Directions and Distribution of Incident Report

1.    Injured party or witness fills out Incident Report, fillable form or a printed copy

2.    If utilizing the fillable form it will not save your document, simply print it

3.    Author gives report to Manager

4.    Manger signs report, scans and sends file to “Incident” group

5.    Manager, Finance Director or Administrative Assistant will save it to the confidential Management folder, under Incident Reports. Saved as Last name and Month of incident.

6.    Leadership Committee will review incident reports submitted.
